Delivery and Shipping

Shipping Time - Most orders will ship in 1-5 business days. Printed items will take longer. In stock items are usually shipped in 1-2 business day.  Orders are not processed or shipped on Saturday or Sunday. Delay in shipping may be because of a complex order, special imprinted order, UPS/FedEx and or delays at the warehouse. We will email you when the order has been shipped by UPS/FedEx. Please check your tracking number for any updates, and or delays.

Required Signature: All imprinted orders/checks/deposit tickets and others will have a Required Signature. UPS/FedEx will only deliver during business hrs. of your business. Required signature protects us/you against fraud orders, and theft.
 
Stolen Packages: Choice Checks is not liable for any stolen packages from your office, porch, warehouse or any other location. We provide a tracking number on each order, and its your responsibility to make sure that someone is there to receive the package. This applies to any item we ship, even imprinted items. If an item is stolen. You'll be required to replace the order.
 
We cannot guarantee when an order will arrive. Consider any shipping or transit time offered to you by this site or other parties only as an estimate. We encourage you to order in a timely fashion to avoid delays caused by shipping or product availability.

Description

  • Each patient prints his/her name, apt/arrival time and "Name of Healthcare Professional" on a numbered line. If using the optional numbered label, patients takes the number. The office staff removes the signed name label and either adds to the patient file or to a workflow tracking system. Once the label has been removed, the information is protected behind part one, available for office use.
  • The front desk attendant calls each patient by number or name (Available in blue, burgundy or green.)
  • Each package contains 125 forms, numbered 1 through 25, or enough forms to see 3,125 patients.
