• Employer Copy D (records)
• Employee Copy B (federal) Copy 2 (state) and Copy C (file) 

Employee Copies B, 2, and C are provided on one 3-up sheet with employee instructions printed on the back (top and bottom forms) and also include a 3/4'' side perforation. The employer 2-up forms are printed with no side perforation. Print each copy as a batch, then fold and mail recipient copies. Mail employer copies in a batch to the SSA. Order by number of forms, not sheets.
